Samsara insta-kill % chance may be wrong in Persona 3[]

I started some skill editing in Persona 3 and, with the help of Dniwe, determined most of the variables in the Persona 3 FES skill.tbl, which contains seemingly all relevant skill data for non-passive skills (passives appear to be hardcoded):

Samsara

In SKILL.TBL (and I assume SKILL_F.TBL for The Answer), Samsara (along with every other insta-kill move in the game) have an accuracy of 100 (for comparison Ziodyne has 95, Maziodyne has 90) and different ailment percent chances which appear to be the actual percent chance of the instant kill landing. Hama is 30%, Samsara is 45%.

This 100% accuracy is before your agility, opponent's agility, buffs, debuffs, and levels are factored in. For example when I set the aiment percent to 255% (highest it can go) and the accuracy to 1%, my lvl 99 max agility Savior was still able to hit Samsara about 50-70% of the time against level 50-ish enemies, and about 30-40% of the time on a lvl 54 persona with about 30 agility (MC was lvl 64 in both cases).

The ailment percent chance is (seemingly) only impacted by passives and elemental affinities. Since all the moves have incredibly high accuracy, passives target the IK percent rather than the accuracy. This needs more testing to confirm, but at the very least there does appear to be two separate calculations that occur before Samsara hits.

The ailment percent appears to be different than official sources for nearly every move in the game, the only correct one I could find was that Holy Arrow has a 25% charm chance, every single target ailment skill is 40% with multi-target being 35%.

So my questions would be, is there a way to check these values against the original P3 release? What about P3P? And is there anything in the damage formula that might also affect the total percentage, such as never being able to hit with the full percentage from these table no matter the enemy with no Light/Dark resistance?
