Punch Ouch is a Retro Game item.

Punch Ouch can be played during the protagonist's daily life. The full game is split into three chapters, with each requiring a single session to be completed; if a chapter is not completed, it will have to be done from the beginning.
The rules of the game involve inputting specific combination of buttons as well as mashing the X button several times, both within a time limit. Those will generally be repeated several times per session.
- The first session is a practice course and it cannot be lost at. The first task is to input <><>X (sway) in 3 seconds, before having to mash the X button (flurry) in the same period. There are given 2 attempts for each, before progressing to the next task. The session is completed with a combo of both: failing the flurry will start the entire combo over, but if the sway is failed once, the next unsuccessful task will prompt the protagonist either starts the entire session over or give up, ending the session.
- Cheats increase the duration time for sway to 5 seconds, while flurries are to 4. (Note: 2nd flurry, if tried all over again, gives 3 seconds only.)
- The second session involves having to sway (3s), flurry (2s), sway (3s) and flurry (3s).
- If any of the tasks are failed, there will be three chances to go back from where one's left off and try them again, but only after pressing the X button several times in 5 seconds: first chance requires 15 presses, the second 20, and the third 30. Failing the general fight 4 times, or failing to recover even once will end the game with a loss.
- If the 2nd flurry is failed, the challenger will be sent back to the 2nd sway, requiring both be finished at once.
- Cheats increase the duration time like so: sway (5s), flurry (5s), sway (5s) and flurry (4s). Recovery time is left unaffected.
- If any of the tasks are failed, there will be three chances to go back from where one's left off and try them again, but only after pressing the X button several times in 5 seconds: first chance requires 15 presses, the second 20, and the third 30. Failing the general fight 4 times, or failing to recover even once will end the game with a loss.
- The third session is different: the sway is replaced with an uppercut, specifically ^^X, and the flurry is more complex, having to press the X button 10 times. The order is as follows: uppercut (3s), sway (3s), uppercut (2s), sway (3s), and a finishing combo of <><>^^X (5s.)
- The recovery rules for the 3rd session are the same, except that the 2nd uppercut, sway and finishing combo have to be done at once: failing either will bring one back to the 2nd uppercut.
- Cheats increase duration as follows: uppercut (5s), sway (4s), uppercut (3s), sway (4s) and finishing combo (8s.) Recovery time is left unaffected.
For each completed session of Punch Ouch, the protagonist will receive two points of Charm, regardless of whether he wins or not. Upon completing the entire game, he will also receive a Boxing Card, a redeemable item.
In Persona 5 Royal, the protagonist will only be prevented from receiving a point loss during his first attempt at playing the game: from the second attempt onward, losing will grant only one point of Charm.
